The associated activities are being undertaken at my own risk and the company / Promotion involved herewith, namely GROUNDQUEST and associates will not  be  liable  to  me  or  my dependents for any claims arising from injuries, whether fatal or otherwise, sustained during an activity or associated activities, how so ever such injury is caused.

The company / promotion and associates will not be held liable for any loss of or damage to any property sustained by me, howsoever such loss or damage caused.

I undertake to comply with the instruction of the company’s employees / Promotion Officials, and trainers.

I am not under the influence of alcohol or any other drug with narcotic effect or that changes my state of mind or any performance enhancing substance as set out in the drug free sport banned substance list.

I hereby indemnify and hold harmless the company / promotion and their associates and all other persons associated with GROUNDQUEST against all actions or claims by me arising from the said associated activities to which I am aware of all the inherent dangers associated with the sport.

I accept that this contract sets out the full basis of the relationship between Promotion and the associates and myself and I acknowledge that it is governed by the laws of South Africa and that in the event of my bringing any legal action against the company / promotion, MMA South Africa, their associates, crew members or contractors, I agree that only the courts of South Africa shall have Jurisdiction.

I hereby indemnify the Municipality, Promotion and GROUNDQUEST and its Officials against and hold it harmless from any claims or damage which may be instituted or suffering by any person,  myself,  including legal  costs  incurred,  as a  result of any loss or injury whether fatal or otherwise, to any person or event or as a result of any failure by me to comply with any conditions contained herein, or instruction given, in the undertaking of activities.

Once the indemnity form has been signed, all activities are non-transferable and non-refundable.

I confirm that I am not suffering from any medical condition listed that may exclude me from doing any activities associated with Full contact Sports. I confirm that I am not using any illicit substances or any form of performance enhancers as set out by the ANTI doping association of South Africa.

I hereby agree to the following  the rules and regulations and code of conduct set out by the promoter and Federation.

Pregnancy, high blood pressure, heart conditions, neurological Disorders, Epilepsy, acute or chronic knee, joint or back disorder, conditions of the skeleton, muscles or nervous system, osteoporosis, serious car accidents, recent operation, dislocation, severe medical conditions, acute medical conditions, muscular conditions.

Any person with declared medical conditions will not be permitted to partake unless a medical certificate approving the activities and signed within the previous 7 days is obtained. The original copy is to be handed in before the activity is undertaken. (If you are uncertain of these conditions, please confirm with the medical staff.)

I understand that there shall be no liability on the part of Promotion, the Competition Medical Team or any other party associated with the Federation should I withhold any medical information.

I consent to the personal and medical information I have submitted to GROUNDQUEST (either directly, or through my team) being held by the Federation and, where necessary, shared with partner organizations for the purposes of compliance with athlete medical safety standards. I understand that to withdraw this consent I must notify GROUNDQUEST in writing.

I have read and understood the SAIDS & WADA Anti-Doping Rules and understand that it is my responsibility to know and adhere to IMMAF Anti-Doping Policy and applicable Prohibited List.

I give my express consent to the processing of my personal data under this policy and consent to the sharing of my  personal  data  with  relevant  officials  and  organizations  so  authorized  to  review  this information. I understand that my participation in grappling events is contingent upon my voluntary participation in anti-doping and medical procedures set forth in policies, requiring the processing of my personal data. I understand that withdrawal of my consent to processing of my personal data shall be construed as a refusal to participate in those procedures which could exclude me from further participation and may result in disciplinary or other sanctions being imposed on me.

Pursuant to applicable laws and anti-doping standards, I have certain rights in relation to my personal data, including access and opportunity to correct inaccurate data. I hereby release GROUNDQUEST, as well as related anti-doping organizations and accredited laboratories from all claims, demands, liabilities, damages, costs and expenses that may arise in the lawful processing of my personal data. I acknowledge and agree that any dispute arising out of the SAIDS Anti-Doping Rules may be, after exhaustion of disciplinary processes as provided for In the SAIDS Anti-Doping Rules, appealed to the Court of Arbitration for Sport.

The decisions of the arbitral appellate body shall be final and enforceable, and I will not bring any claim, arbitration, lawsuit or litigation in any other court or tribunal.
